Our services are available to individuals of all ages. Minors (persons under 18) may only receive services with the consent and active involvement of a parent or legal guardian. By using our services, you represent that you have the legal capacity to enter a binding agreement and that all information you provide is accurate and complete.

Consent to Telehealth

By scheduling or participating in a telehealth visit, you consent to receiving healthcare services via electronic communication. You understand that this may involve the electronic transmission of your medical information. You may withdraw consent to telehealth at any time by notifying us in writing, after which alternative arrangements will be discussed.

Technology Requirements

You are responsible for ensuring you have the necessary technology, internet connection, and a private, appropriate environment to participate in telehealth visits. We are not responsible for service interruptions caused by your equipment, internet service provider, or telecommunications infrastructure.

Clinical Limitations

Telehealth visits have inherent limitations compared to in-person care. Your provider may determine that your condition requires an in-person evaluation and may decline to continue a telehealth visit if clinically appropriate. Such determinations are made solely in the interest of patient safety.

Appointments must be scheduled in advance. We will make reasonable efforts to honor scheduled times; however, mobile services may be subject to delays due to traffic, prior patient needs, or other circumstances beyond our control. We will notify you of significant delays as promptly as possible.

Cancellations or rescheduling requests must be made at least 24 hours prior to your scheduled appointment. Repeated no-shows or late cancellations may result in additional fees or suspension of scheduling privileges at our discretion.

Insurance Billing

We accept a variety of insurance plans. It is your responsibility to verify that our services are covered under your specific plan. You authorize us to bill your insurance provider on your behalf and to receive payment directly from your insurer. You are responsible for any copayments, deductibles, or non-covered services.

Self-Pay

For patients without insurance coverage or for services not covered by insurance, payment is due at the time of service unless otherwise arranged in advance. A fee schedule is available upon request.

Billing Disputes
